• Fill out the HHCAHPS consent form (available at https://homehealthcahps.org/) and log in and update your passwords.
• On that website, HHAs will need to authorize Communications for Research, Inc. as their vendor to allow us to submit data on your behalf.
• Provide CFRI with your CMS Certification Number (CCN, previously known as Medicare Provider ID Number) and legal Name of the HHA to be used.
• Sample (information about patients) must be sent to CFRI every month. We need to receive it on or before the 10th of each month.

When sending patient information, you may send it password protected and/or encrypted to ensure confidentiality.

Information, on all of the patients that received service in that calendar month, should be sent to CFRI and we will sort through who is eligible to call.

CFRI will then take care of everything else for you. Including sending your data to CMS.
