Getting your HCAHPS survey started with CFRI Print

 

  • Sign up with QualityNet.org to allow us to submit your hospital’s data. CFRI can provide this form to your hospital with our company information already filled out; your hospital will just need to sign and mail in.
  • Provide CFRI with your hospital’s NPI, MPN (also called CMS certification number or CCN), and legal name of the hospital to be used with the records.
  • Provide CFRI with the name and number of a person whom patients with questions about the legitimacy of the survey can contact at the hospital.
  • Sample (information for discharges) must be sent every month. This can be sent once a week, once every two weeks or once a month, but we need to receive it on or before the 5th of the month following the discharges (for example, January discharges must be sent by the 5th of February).

When sending patient information, hospitals may send it password protected and/or encrypted to ensure confidentiality.

Data from all of the patient discharges should be sent to CFRI and we will sort through who is eligible to call.

 

CFRI will then take care of everything else for you. Including sending your data to CMS.
